Patient Portal and Office Forms

At Accent on Vision ABQ, we use DigiSign to email our office policy forms to our patients prior to their appointment. Our office forms can be signed digitally and automatically returned to our admin team. Below you can find information about our patient portal and office forms. Coordination-of-Benefits (COB): Certain diagnoses, such as Diabetes, Glaucoma, and others, are not covered by vision insurances and must be billed to medical insurance. Medical insurance may not cover 100% of the services and most vision insurance plans do not allow a coordination-of-benefits claim to be filed. This would allow us to bill both medical insurance and vision insurance on the same day. There are a select few vision plans that will allow us to bill a COB claim to them. In the event your vision insurance plan does not allow a coordination-of-benefits there are a couple of options that you have. Our admin team will let you know of your COB options via emailed forms or when you check in for your appointment. You may reach out anytime with any questions regarding your COB status.

Patient Portal: Our patient portal is designed for ESTABLISHED PATIENTS ONLY. This is where they can update their demographic info, ocular and medical history, medications list, and more, before their appointment. This allows our team to properly prepare for the appointment ahead of time. If you are a new patient and would like to establish care with us, please give our office a call to schedule.

Optomap/Financial/Contact Lens Service Policy Form: We require this form to be reviewed and signed annually for every patient. This form provides information about exam fees, required imaging for every patient, and fitting fees for contact lens patients.

Refraction Form: A refraction is the clinical term for evaluating a person’s vision to determine if they need a prescription for glasses. This evaluation is NOT covered by medical insurances, except for a few specific plans.

Records Release Form: A records release form is required if a patient wants their exam records to be provided to them or to another office. This ensures maximum protection of sensitive information.  Click Here to access the release form.
